The National Transportation Safety Board has released a preliminary report on the fatal crash of a single engine air tanker (SEAT) in Idaho.
The Air Tractor AT-802A crashed September 22, 2020 while working on the Schill Fire, approximately 2 miles southeast of Emmett.
The pilot, Ricky Fulton, perished. The aircraft, Tanker 857, was owned by Aero S.E.A.T. Incorporated and was on an on-call (CWN) contract with the Bureau of Land Management. The aircraft was first registered July 10, 2020, FAA registration number N836MM.
Typically it takes 8 to 16 months for the NTSB to issue their final, complete report with an analysis of the causes of a crash.
This was the sixth firefighting pilot and the third SEAT pilot to be killed in the United States this year. In addition, three members of the crew of a C-130 from the U.S. died when their air tanker crashed January 23, 2020 while fighting a bushfire in New South Wales, Australia. Also, one person was killed August 8 in the crash of a CL-215 based in Portugal while battling a fire in Spain.
Below is the complete text from the narrative section of the report about the September 22 crash.
On September 22, 2020, about 1830 mountain daylight time, an Air Tractor AT-802A, N836MM, was substantially damaged when it was involved in an accident near Emmett, Idaho. The pilot was fatally injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 aerial firefighting flight.
Witness conducting firefighting operations, adjacent the accident site, reported that the accident airplane, a single engine air tanker (SEAT), descended and made an approach similar to the previous SEATs that were dropping fire retardant. The witnesses said the airplane passed over the top of the ridge and descended into the valley, however, the pilot did not drop the fire retardant as previous SEATs did. The witnesses stated he heard a brief application of engine power as the airplane began to ascend over rising terrain at the pilot’s 12’oclock position. The airplane subsequently impacted rising terrain near the peak of the ridgeline.
A video provided by a witness captured the accident sequence. The recording showed the airplane descend over an intermediate ridgeline and into a valley (see figure 1). About 3 seconds later, the airplane momentarily returned to level flight before it pitched to a nose-high attitude. The airplane subsequently impacted rising terrain approximately 80 feet below the ridgeline.
Examination of the accident site by a Federal Aviation Administration inspector revealed that the airplane impacted rising terrain. The wreckage debris path continued from the initial impact point over the top of a ridgeline, and extended into a small ravine. The airplane came to rest approximately 100 yards from the initial impact pointe on a heading of 040 degrees. All major structural components of the airplane were located throughout the wreckage debris path. The wreckage was recovered for further examination.
If you are like me it seems like there have been a lot of deaths of firefighting pilots recently (there have been) and maybe it is getting difficult to keep track. I feel bad if for a moment I can’t remember all six men. So to help me and anyone else that can benefit from having an (awful) list to refer to, here is a brief summary, with links to articles that have more details.
During the 49-day period that began July 7 there were six crashes of firefighting aircraft — three helicopters and three air tankers. In chronological order, they are:
July 7 Bryan Jeffery Boatman, 37, was approaching a helispot delivering supplies by long line to firefighters in a remote area on the Polles Fire in central Arizona. The UH-1H was operated by Airwest Helicopters out of Glendale, Arizona. The aircraft was under an exclusive use contract with the USFS.
July 30 Two single engine air tankers (SEATs) crashed after a mid-air collision while working the Bishop Fire in southeast Nevada. The pilots were David Blake Haynes and Scott Thomas. Both Air Tractor AT-802A aircraft were operated by M&M Air Services out of Beaumont, Texas on a BLM contract.
A Portuguese water-scooping air tanker, a CL-215, crashed in Spain August 8 while battling a wildfire that started near Lindoso, Portugal and burned across the international border. The pilot, Jorge Jardim, 65, was killed and the Spanish co-pilot was seriously injured. The scooper was operated by the Portuguese branch of the international company Babcock.
August 19 Mike Fournier, 52, was killed in the crash of a Bell UH-1H helicopter while on a water dropping mission on the Hills Fire, approximately 9 miles south of the City of Coalinga. It was operated by Guardian Helicopters out of Van Nuys, California on a CAL FIRE contract.
August 24 Tom Duffy, 40, died in a helicopter crash during a water bucket mission on the White River Fire in Oregon. The K-MAX was operated by Central Copters of Bozeman, Montana on a Call When Needed contract with the U.S. Forest Service.
As a former firefighter I don’t view all firefighters as heroes, of course, but we should honor these men for their service, and pray that their families can find some sort of peace knowing that they were on a good, honorable mission in life, helping others and doing things that few people can.
The Sheriff’s Office of Lincoln County, Nevada has officially released the names of the two pilots that were killed in the mid-air collision of two Single Engine Air Tankers that occurred July 30, 2020 that were working on the Bishop Fire in southeast Nevada. The aircraft were on contract with the BLM.
They were identified by the Lincoln County Sheriff’s Office as David Blake Haynes and Scott Thomas. When we asked Sheriff Kerry Lee why it took almost three weeks to release the names, he said that they have four ways to identify deceased individuals — dental, fingerprint, identification by the family either on scene or by photo, or DNA. Because of the condition of the victims they had to use DNA which required obtaining samples from family members then sending those to a lab with samples from the pilots.
The investigators found that the tankers were working in tandem with one close behind the other. After the following aircraft got retardant on the windshield it made a rapid climb then suddenly turned left and collided with the other.
Both of the SEATs were operated by M&M Air Services out of Beaumont, Texas. The aircraft were made by Air Tractor, model AT-802A; N8510M (Tanker 866) and N1558W (Tanker 824).

In the last few weeks the U.S. Forest Service has brought on ten additional air tankers on a temporary basis. This includes CL-415 water-scoopers, CV-580’s, and Modular Airborne FireFighting System (MAFFS) C-130’s. Two of the aircraft were acquired through Call When Needed (CWN) agreements; four via agreements with Alaska and Canada; two MAFFS through an agreement with the Department of Defense; and two water scoopers through other contracts.
The U.S. Forest Service expects to have two HC-130H aircraft at McClellan Air Field in September. These are part of the seven aircraft fleet of HC-130H’s that the agency is receiving from the Coast Guard.
Last year one of the HC-130H’s worked out of McClellan using a MAFFS, a slip-in 3,000-gallon pressurized retardant system that pumps the liquid out the left side troop door. That was aircraft #1721 designated as Tanker 118, still painted in military colors. T-118 is now undergoing scheduled depot-level maintenance and should be replaced in September of this year by #1708 designated as Tanker 116. It will also use a slip-in MAFFS unit, one of the eight owned by the USFS, but should be sporting a new USFS air tanker paint job. After T-118 left, another former Coast Guard aircraft took its place, #1706. It is being used for training the contracted pilots and will not serve as an air tanker.
Eventually the USFS hopes to have all seven converted to air tankers with removable retardant tanks. A contract for the installation of the retardant delivery systems was awarded to the Coulson Group in May. There is also much other work that has to be completed on the aircraft including programmed depot maintenance, painting, and wing box replacement on most of them. The work is being done or coordinated by the U.S. Air Force. They were directed by Congressional legislation to use their own funds, up to $130 million, so it is no surprise that the schedule keeps slipping as delays continue to occur in awarding contracts and scheduling the maintenance.
The USFS has two water-scooping CL-415 air tankers on exclusive use contract. As noted above they recently temporarily brought on two more on a call when needed basis. All four are operated by AeroFlite and as seen in the photo above were together at Cody last week.
There was some discussion in the comment section of another article on Fire Aviation about the status of the BAe-146 aircraft being converted to air tankers by Air Spray. The company has five of the 146’s; two are out of the country and the other three are at the company’s Chico, California facility. Ravi Saip, their Director of Maintenance/General Manager, told Fire Aviation that they expect to begin flight testing one of them in air tanker mode around the first of the year. After they receive a supplemental type certificate from the FAA, work on the second one would shift into high gear. Then conversion of the other three would begin.
Air Spray also has eight Air Tractor 802 single engine air tankers that they have purchased since 2014. Five of them have received the amphibious conversion by adding floats, and the other three are stock, restricted to wheels.
Air Spray’s Tanker 498, an L-188 Electra, is currently in Sacramento being inspected and carded by CAL FIRE so that it can be used in a Call When Needed capacity.
Jim Wheeler, President and CEO of Global SuperTanker Services, told us that the FAA has awarded a supplemental type certificate for their reborn 747 SuperTanker — a major and sometimes very difficult barrier to overcome. Within the next two weeks they expect to receive the airworthiness certificate.
Beginning next week representatives from the USFS will observe some additional static tests and then there will be an airborne descent test, a new test added in 2013, releasing retardant in a downhill drop. That test was not required when Version 1.0 of the 747 was certified. It may have been added after it was discovered that the first BAe-146’s that were converted and issued contracts still retained hundreds of gallons of retardant after downhill runs.
These steps should take less than two weeks, Mr. Wheeler said, after which they will submit the results to the Interagency AirTanker Board.
Jennifer Jones, a spokesperson for the USFS, told Fire Aviation that the company was offered an opportunity to submit a proposal for a call when needed contract solicitation in 2015, along with numerous other companies, but declined to do so. Their next opportunity to obtain a contract will be when another general solicitation is issued in 2017, or perhaps sooner, Ms. Jones said. The agency issued a Request for Information a few weeks ago, which is usually followed some months later with an actual solicitation.
Judging from the list of CWN air tankers with contracts, apparently it is possible to submit a proposal and receive a USFS CWN contract even if the aircraft exists mostly on paper and could be years away from being FAA and Interagency AirTanker Board certified.
In the meantime Mr. Wheeler realizes that the USFS is not the only organization that hires air tankers and has been talking with a number of other agencies in various states and countries as well as companies involved in marine firefighting.
Global SuperTanker is in the process of finishing repairs on the 747 in Arizona after some of the composite flight control surfaces (flaps, spoilers, elevators) and engine cowlings were damaged by golf ball sized hail at Colorado Springs several weeks ago. There was no windscreen or fuselage damage.
Mr. Wheeler said that was the first severe hailstorm within the last seven years at the Colorado Springs airport. But, after the aircraft left to be repaired in Arizona a second hailstorm struck the airport that some have said was a 100-year event and did much more damage than the first one.
Since then no decisions have been made. Ms. Jones told Fire Aviation:
The U.S. Forest Service continues to cooperate with the Department of Defense to identify potential federal facilities, which must be considered first.
It is unlikely that more than one or two of the seven HC-130H’s would be at the new base at at any one time, except during the winter when they would not have to be dispersed around the country to be available for firefighting. While the base might not be a huge expansion of the aerial firefighting capabilities in an area, the stationing of the flight crews, maintenance, and administrative personnel would be a boost to the economy of a small or medium-sized city.
As we are writing this Saturday evening, a single engine air tanker (SEAT) is en route from the United States to Australia. An Air Tractor 802, Tanker 849, N30723, departed from Santa Maria, California on September 30 at 8:33 a.m., and after stopping in Honolulu and the Marshall Islands is now on the way to Honiara, Guadalcanal with an expected arrival time of 9:12 p.m. MT, October 3.
On the 13-hour leg to Honolulu it flew at 5,000 to 8,000 feet at a ground speed of 175 to 219 mph.
We have heard but can’t confirm that fuel can put in the hopper (retardant tank) on a SEAT to extend the range. Obviously extra fuel is stored somewhere to enable the 13-hour flight.
We found out today that the SEAT was operated this summer by Aero Spray, a company headquartered in Appleton, Minnesota that operates several Air Tractor aircraft for spraying crops and fighting wildfires. They were the first company in the United States to operate an Air Tractor on floats. Laura at the company told us that they leased the Air Tractor Fire Boss, Tanker 849, from Pays Air Service in Australia, and it is being returned to the owner.
In an earlier comment on this article, Bean said the Air Tractor website mentions a ferry kit that enables 800 gallons of fuel to be carried in the hopper. And at Airliners.net a person identified as “AT502B” wrote:
AT 802’s are available with a ferry kit, which enables you to fill the water tank-or Hopper- up with fuel, thus giving you an additional 800gal of fuel. Add this to the standard fuel tank size of 336us gal- gives you some serious fuel capacity for a single engine aeroplane. An AT 802 burns approx. 75-85us gal per hour at a ferry speed of 200mph- so it gives you a pretty good range as well.
The aircraft arrived in Queensland, Australia Sunday, October 4 at 7:53 p.m. MT according to Flight Aware, after a flight of almost seven hours from Guadalcanal.
An engine failure on Conair’s amphibious Air Tanker 699, an Air Tractor AT-802A, during training resulted in damage to a float upon landing. The incident occurred April 11, 2015 on Harrison Lake, BC, 33 nm NNE of Abbotsford.

…The aircraft was a 2-seat model conducting the fourth touch and go exercise. On the departure, the trainee was given a simulated engine failure at 700 feet above the surface. Following the emergency drill, the training pilot requested a go-around at 400 feet. When engine (PT6A-67F) power was commanded, flames were observed coming from the exhaust stacks and the engine was reduced to idle power and shut down. The emergency landing was carried out straight ahead but was hard and a bounce resulted in float damage. The FCU [fuel control unit] will be torn down for examination at the manufacturer’s facilities.
